You can apply to become a patient by filling out our “Patient Registration form” to schedule an initial appointment.
You will need the following documents to complete your registration electronically. If you do not have a specific document, you can also provide a letter of support indicating where you live and who is financially supporting you.
- Copies of current photo ID
- Utility bill, lease, or any mail postmarked or dated within 30 days (or letter of support)
- If employed, current and consecutive pay stubs for the last 30 days (or letter of support)
- If you file taxes, Federal Income Tax Returns (Federal 1040 form) for you and your spouse
General Eligibility Criteria
We provide medical, dental, pharmaceutical, and behavioral health services to adults who reside in Roanoke or the surrounding area who are uninsured or underinsured, and also have an income up to 300% of the Federal Poverty Level. Individuals are considered underinsured if their health insurance does not cover the medical service they are requesting. Anyone can participate in the two Educational Classes we offer as well!
We provide healthcare for:
- Individuals and families with an income up to 300% of the Federal Poverty Level – For example: Single individuals who make $37,470 a year or less or a family of three earing $63,990 for a family of three. The qualifying income varies depending on the number of people in your household.
- People who are uninsured or underinsured.
- Patients who are covered by any form of Medicaid
- Individuals who are employed or unemployed or serving as caregivers
- Residents of the Roanoke Valley and surrounding areas
Service Limitations
Services at BFC are primarily provided by volunteer nurses, physician assistants, dentists, and physicians. Therefore, we are unable to provide certain services. In these circumstances, we can provide outside referrals to other free or low cost health care providers to treat you!
